Nebraska's Medicaid NEMT mileage reimbursement rate hasn't kept pace with fuel, insurance, and vehicle maintenance costs — squeezing margins for the small operators who make up most of the state's rural NEMT network. NANEMTP is compiling real cost data from member fleets to bring to DHHS and MTM ahead of the next rate review, and we need providers willing to share their numbers.

On May 1, 2026, Nebraska became the first state to implement strict Medicaid work requirements, mandating 80 hours/month for enrollees aged 19–64. Healthcare advocates warn of a 10–15% enrollment drop — meaning fewer eligible members and a direct reduction in NEMT trip volume. NANEMTP recommends all providers verify member eligibility through broker portals before dispatching to avoid unpaid trips.
Nebraska DHHS Division of Medicaid and Long-Term Care (MLTC) has issued a formal public notice updating outpatient and professional provider payment rate structures effective July 1, 2026. These annual shifts dictate fee schedule weights for the next 12 months. NANEMTP will be reviewing the final schedules to ensure NEMT mileage and base rates keep pace with rising fuel and vehicle maintenance costs in Nebraska.
MTM Health now manages transportation for UnitedHealthcare Community Plan, Molina Healthcare, and Nebraska Total Care — commanding the vast majority of Nebraska's Medicaid transit network. While streamlined, this leaves independent operators vulnerable to system-wide policy changes and strict auditing. NANEMTP is gathering member feedback to present unified concerns directly to MTM regional leadership.
